Her suspension is due to end on 21 June 2030.
Cas verified to BBC Sport that it had received the appeal but mentioned a hearing date had not yet been set.
Vondrousova, who in 2023 became the first unseeded player to win the Wimbledon women’s singles title, mentioned in April that when she refused the test she was at “breaking point after months of physical and mental stress”.
The Czech added that she feared for her safety when the officer called at her home at 20:00 on 3 December and mentioned they failed to properly identify themselves or follow “protocol”. She signed a refusal form when she went outside to walk her dog shortly afterwards.
However, a tribunal determined Vondrousova provided “no compelling justification” for refusing the test, nor to receive a shortened ban.
